1. Introduction

2. Appointments and Cancellations
2.1 Booking Appointments:
- Appointments can be made online appointment, or in person. Accurate personal information must be provided.
2.2 Cancellation Policy:
- In case of online booking Patients there is no cancellation of booking and fees are non refundable it can be rescheduling of appointments depends upon the management jurisdiction . Failure to do so may result in a cancellation fee.
2.3 No-Show Policy:
- Patients who do not show up for their scheduled appointment without prior notice may be charged a no-show fee.
3. Consultations and Treatments
3.1 Consultation:
- Patients must provide complete and accurate medical history and information during consultations. This is crucial for accurate diagnosis and treatment planning.
3.2 Treatment Plans:
- Treatment plans will be discussed with patients. Patients are encouraged to ask questions and fully understand the proposed treatments before proceeding.
3.3 Patient Compliance:
- Patients are expected to follow the treatment plans and advice provided by their healthcare providers to achieve the best outcomes.
4. Medical Records and Privacy
4.1 Confidentiality:
- All patient information and medical records are kept confidential in compliance with applicable laws and regulations.
4.2 Access to Records:
- Patients have the right to access their medical records. Requests for access must be made in writing and may be subject to a processing fee.
4.3 Data Protection:
- Akriti Clinic employs appropriate technical and organizational measures to protect personal and medical data against unauthorized access, alteration, or destruction.
5. Payment and Billing
5.1 Payment Terms:
- Payment for services is due at the time of consultation or treatment. We accept cash, credit/debit cards, and online transfers.
5.2 Insurance:
- Akriti Clinic accepts certain health insurance plans. It is the patient’s responsibility to understand their insurance coverage and any costs not covered by insurance.
5.3 Refund Policy:
- Refunds for prepaid services will be considered if services are canceled at least 24 hours before the scheduled appointment. Refund requests must be submitted in writing and will be processed within 30 days.
6. Liability
6.1 Limitation of Liability:
- Akriti Clinic, its staff, and affiliates are not liable for any direct, indirect, incidental, or consequential damages arising from the use of our services.
6.2 Indemnification:
- Patients agree to indemnify and hold harmless Akriti Clinic from any claims, damages, or expenses arising from their use of the clinic’s services.
7. Governing Law
These terms and conditions are governed by and construed in accordance with the laws of India. Any disputes arising from these terms shall be resolved in the courts of Dhanbad, Jharkhand.
